How long before anything changes?
Local work usually starts showing movement in 30 to 60 days if your site already has some history. A brand new site takes longer, closer to four to six months, because there is less for Google to trust. Anyone who tells you two weeks is selling you something.
What do you actually do every month?

Do I have to do anything?

How I handle the medical side
I write nothing clinical. No claims about results, no claims about safety, nothing about what a treatment does. Google holds health content to a higher standard and aesthetic advertising is regulated separately, so anything touching medicine goes to your clinician for sign off and publishes in their name. That keeps you out of trouble, and it happens to be what search engines reward anyway.
I also do not run your Instagram. Different job, different person.
